I acknowledge and consent that any air monitoring test results and location information may be put on the City’s website or otherwise be disclosed to the public. I understand that this consent is voluntary and may be revoked in writing at any time prior to entry.

By electronically signing this form, I grant permission to representatives, employees, or contractors of the City of Galena, KS, to access the specified property for the purpose of conducting air monitoring. I acknowledge that this document and the associated testing results may be subject to the Kansas Open Records Act (KORA).
